| Those who "walk-in " without an appointment create confusion and interfere with those individuals who have called and do have appointments. |

| We ask that you call in advance to arrange for an appointment when in need of medical care. If you have a medical emergency and go the emergency room, please call our office so that we may notify your insurance company of your emergency department visit. |

| If
you believe you are having a life threatening medical emergency go directly to
the Emergency Department and call us later as per your insurance carrier's
instructions. |

Prescription Refills:
Also please note
that the fastest way to get a refill on your prescriptions is to call your
pharmacist, who will then fax a request to us.
Holiday Closings:
In 2006 our holidays will be New Years, Memorial Day,
Independence Day, Labor Day, Thanksgiving and Christmas. Our office will be closed these days.
| In the case of inclement weather please contact our office before coming to your appointment. We consider the safety of our patients and our staff, and evaluate the condition of the roads and the degree of icing of the parking lot before we decide on the hours that Centennial Medical Group will be open each day. There is always a physician available by phone to care for emergencies even in the event of inclement weather.

| New Patients to our practice are requested to complete a new patient questionnaire
and a registration form and bring it with them to their first visit. As well,
you will need to bring your insurance card and a picture identification card. You can print out the
New Patient Questionnaire
and Registration form
from this web site, or ask that
it be mailed to you if you wish. Please also request your prior medical
records if possible and bring them with you to your first visit. The practice
is currently accepting new patients. |

| Our Hours of Operation
are Monday through Friday from 9 A.M. to 1 P.M. and 2 P.M. until 5 P.M. for
patient care, and from 8:30
A.M. to 1:00 P.M. and from 2:00 P.M. to 4:30 P.M. for phone calls.
(except Wednesday when phones reopen at 2:30 P.M.) |

Admissions: When necessary, patients of the Centennial
Medical Group are
admitted by the physicians of the practice to
Howard
County General Hospital, a member of Johns Hopkins Medicine,
in Columbia, Maryland.
