Mail Order Prescription Reminder

Beginning January 1, 2006 we will process Mail Order Prescriptions by giving the patient the prescription to send to their mail order company personally.  Any additional forms or paperwork related to the prescription will become the responsibility of the patient.

The demands to use the mail order forms have become far too time consuming and are impacting our ability to provide a timely turn-around.  The calls from the mail order companies telling us we didn’t fax them their forms when we did is also on the rise.  All of this takes time away from patient care which is unacceptable.

We will be happy to mail your prescription to you or you are welcome to call ahead and have it available for pick-up at the Front Desk.   Please call a day ahead if you plan to pick up your prescriptions.

The mail order company can provide you, the patient, with the forms for the detailed completion they require.  It is typically personal and demographic information and I.D. / Plan numbers.  You can complete the forms and mail the prescription along with the forms they require so you can be assured your prescription has been processed.

We apologize for any inconvenience this may cause but we cannot have this one process have such a negative impact on the care we provide.

Thanks again for your understanding.
