         Welcome to our office which is  located at 8186 Lark Brown Road, Elkridge MD 21075,(across Route 108 from the Columbia Restaurant Park). Please   click here for  directions to the  office. The Chartwell Medical Center and Pharmacy  building  offers Primary Care (Internal Medicine and  Family Medicine), Laboratory Services,  Physical Therapy, Feldman's Pharmacy,  a Gastroenterology Procedure Center, and offices of many Medical and Surgical Specialists all at one convenient location.

You may also now click here to contact us online to request non-urgent appointments, referrals and to ask simple questions. Please note that for all urgent medical concerns you must contact the office by phone. The best way to  get a refill on your prescriptions is have your Doctor write your prescription at your office visit.  If you need a refill between office visit  call your pharmacist to have them  fax a refill request to the office.  The pharmacist has the numbers needed to perform the refill task.

    Centennial Medical Group is made up of Internists (Drs. Steven Geller and Joseph Gibbons), Family Practitioners (Drs. Richard Kolodrubetz, Kathy York-Jordan, Walter Chi and Rajiv Dua), Certified Registered Nurse Practitioners (Susanne Gibbons and  Amy Dominguez).  We are proud of our caring and competent clinical and administrative staff.

     Centennial Medical Group is very technologically advanced. Our  electronic medical record “ Praxis-EMR” resides  on computers found throughout the practice, including one in each exam room. This  technology allows us to provide  the highest quality of care possible.   Information is readily available that is specific to you medical history in your medical chart and clinical resources are available from various resources using our interconnected system. ALL  records are  kept safe through multiple sophisticated security methods – no access to your records is available to ANY unauthorized person or system.

    The goal of the Centennial Medical Group is to provide  "state of the art" health care in a timely manner to all of our patients.     Centennial Travel Health is a division of Centennial Medical   Group, which in association with Paramedical Training and Services Inc.,  provides medical information and necessary vaccines for  foreign travel. Patients  should  be  aware  that  vaccinations for foreign travel are not paid for by their medical insurance in almost all cases. 

       New Patients to the practice are requested to complete a  New Patient Questionnaire, a Registration form and to bring their previous medical records  to their first visit if possible. These   two forms can be printed from our web site by clicking above or mailed, if requested.

       In 2008 our holidays will be New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas Eve and Christmas Day. Our office will be closed these days.
