     Our Phone Number  is  410-730-3399. On weekends and after usual business hours,  our answering service number     for urgent concerns    is  1-888-501-9719. You may also now click here to contact us online to request non-urgent appointments, referrals and to ask simple questions. Please note that for all urgent medical concerns you must contact the office by phone. The best way to  get a refill on your prescriptions is have your Doctor write your prescription at your office visit.  If you need a refill between office visit  call your pharmacist to have them  fax a refill request to the office.  The pharmacist has the numbers needed to perform the refill task. Effective January 1, 2010, you will be required to present your insurance card at every visit.

    Centennial Medical Group is made up of Internists (Drs. Steven Geller and Joseph Gibbons), Family Practitioners (Drs. Richard Kolodrubetz, Walter Chi and Rajiv Dua), and Certified Registered Nurse Practitioners (Susanne Gibbons, Sonja Barsky and Sabrina Wells).  We are proud of our caring and competent clinical and administrative staff.

     Centennial Medical Group is very technologically advanced. Our  electronic medical record “ Praxis-EMR” resides  on computers found throughout the practice, including one in each exam room. This  technology allows us to provide  the highest quality of care possible.   Information is readily available that is specific to your medical history in your medical chart and clinical resources are available from various resources using our interconnected system. ALL  records are  kept safe through multiple sophisticated security methods – no access to your records is available to ANY unauthorized person or system.

    Several of our providers speak other languages as well as in English:

 Dr. Walter Chi - Mandarin, Susanne Gibbons  CRNP - German and Sonja Barsky CRNP - Spanish. Please make appointments with these specific providers if you feel you would be best understood in one of these languages.

    The goal of the Centennial Medical Group is to provide  "state of the art" health care in a timely manner to all of our patients. Please   feel free to offer any constructive suggestions you might have as  the recipient of the services we provide. We are always looking  for ways to improve our services.

           New Patients to the practice are requested to complete a  New Patient Questionnaire, a Registration form and to bring their previous medical records  to their first visit if possible. These   two forms can be printed from our web site by clicking above or mailed, if requested.

       In 2010 our holidays will be New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas Day. Our office will be closed these days.
