Centennial Medical Group


The Centennial Medical Group is a "By Appointment Only" medical practice. Those who "walk-in " without an appointment create confusion and interfere with those individuals who have called and do have appointments. We ask that you call in advance to arrange for an appointment when in need of medical care. If you have a medical emergency and go to the emergency room, please call our office so that we may notify your insurance company of your emergency department visit. If you believe you are having a life threatening medical emergency go directly to the Emergency Department and call us later as per your insurance carrier's instructions. Prescription Refills: Please request all refills from your pharmacy. The pharmacy will in turn fax the refill request to your doctor for authorization. This process eliminates possible medication errors. Holiday Closings: In 2012 we will be closed for the following holidays: New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas Day and New Years Day. Inclement Weather: In the case of inclement weather please contact our office before coming to your appointment. We consider the safety of our patients and our staff, and evaluate the condition of the roads and the degree of icing of the parking lot before we decide on the hours that Centennial Medical Group will be open each day. There is always a physician available by phone to care for emergencies even in the event of inclement weather. New Patients: New Patients to our practice are requested to complete a new patient questionnaire and a registration form and bring it with them to their first visit. You can click here to complete your forms online. As well, you will need to bring your insurance card and a picture identification card. You can print out the New Patient Questionnaire and Registration form from this web site, or ask that it be mailed to you if you wish. Please also request your prior medical records if possible and bring them with you to your first visit. The practice is currently accepting new patients. Hours of Operation: Our Hours of Operation are Monday through Friday from 8:30A.M. to 1 P.M. and 2 P.M. until 5 P.M. for patient care, and from 8:30 A.M. to 1:00 P.M. and from 2:00 P.M. to 4:30 P.M. for phone calls. Hospital Admissions: When necessary, patients of the Centennial Medical Group are admitted by the physicians of the practice to Howard County General Hospital, a member of Johns Hopkins Medicine, in Columbia, Maryland.
